Below is a link you may download while doing a parent-teacher conference, and below that is the actual form I am going to discuss. It is imperative for parents and teachers to build a good relationship so they can collaborate their ideas together so the student can have great academic success. Some Parent-Teacher Conferences is just time for parents and teachers to talk about the progress of the student and how well the student is doing. Although those conferences are great to have and are productive, most Parent-Teacher Conferences have to do with a problem the student is having in a classroom. In the example below, I have a student that is hyperactive and displays compulsive behavior. It is my job as a teacher to seek out why the student is acting this way and I make the suggestion of a possible IEP meeting with the parents and other professionals to see if the student has ADHD, but during the meeting, I might discover that the parents are divorced and that one parent allows him to be a free spirit and act how he wants and the next parent is very strict, so Raheim just waits to go to school to act however he pleases. I make a compromise with the the parents, but I still recommend an IEP meeting so that I can set-up the classroom better based on Raheim's needs. It is important for the teacher and parents to both be open to hear new ideas because they both can benefit from a Parent-Teacher Conference.
